To me this song is most definitely about addiction. Whether it be drugs, sex, violence, or anything else,there are clearly extremely strong metaphors in both the song and video that would agree. I.e. When the man at the beginning is eating what appears to be razor blades and the lyrics sound "I don't want it I just need it!"...These lyrics also arrive when the woman's addiction causes her to "pull the plug" on her incomplete child.

Addiction often causes us pain and human error. "Elbow deep inside the borderline. Show me that you love me..." our addictions speak to us until suffering. Really though that's only my personal opinion. Interpret it however you like. If anyone honestly has had experiences with anal fisting and this song reaches them in that way, good for you. However, don't judge a book by it's cover.

In terms of drug addiction, possibly a heroin needle. It may hurt a little but it's something you'd get used to once your addicted. This seems likely especially considering a lot of content on this album has been confirmed by Maynard being about his struggles with heroin and his ultimate victory over the drug. On a much broader scale. One realizes the pain any addiction causes at some point, but if the addiction is not conquered the pain goes away and everything that really matters is lost.

i think it's based on humans sensual feelings toward pain, some people who are emotionally disturbed do not create pain because they are depressed, but simply because they are lacking attention, or want to have this pain, just because it supposedly gives a rush.
(E.G) when the man swallows the nails. the song later states finger deep in the border line. this is that this guy has passed the normal emotional cutting line, and is just trying to feel something new.
